By submitting this authorization, I confirm that the information provided is accurate and complete to the best of my knowledge.

I acknowledge that paper pay stubs will no longer be provided. I understand that my pay stubs will be available electronically through Intuit’s Workforce app and that I am responsible for accessing and reviewing my pay information through the app.

I authorize my employer listed above to send credit entries, as well as appropriate debit and adjustment entries, electronically or by any other commercially accepted method, to the account indicated above. This authorization also authorizes the financial institution holding the account to post all such entries.

I agree that the ACH transactions authorized herein shall comply with all applicable U.S. laws and regulations.

This authorization will remain in effect until the Company receives written notice of termination from me and has had a reasonable opportunity to act on such notice.
